Factors Influencing Quality of Caregiving by Caregivers for Elders with Dementia

PURPOSE
The aim of the study is to determine factors influencing quality of caregiving by caregivers for the elderly with dementia.
METHODS
Data were collected from 87 caregivers for elders with dementia who had visited in Busan Metropolitan Center for Dementia and D-University hospital outpatient center from July 10 to September 30, 2015. A self-reported questionnaire was used to assess the severity of the elders' dementia and knowledge of dementia, burdens and quality of caregiving by the caregivers. The SPSS 21.0 version program was used for data analysis. Data were analyzed using descriptive statistics, Pearson's correlation, t-test, ANOVA and multiple regression.
RESULTS
Significant predictors of quality of caregiving by caregivers included caregivers' burdens (explanation power 25%), knowledge of dementia (explanation power 4%) and levels of education (explanation power 3%). These factors explained 32.3% of the variances in quality of caregiving.
CONCLUSION
Burdens on caregivers were a major factor that decreased quality of caregiving, and knowledge of dementia was a factor that increased it. These findings show that educational programs and intervention for reducing burdens and improving knowledge of dementia are necessary to improve quality of caregiving by caregivers.

PURPOSE
To explore the association between sleep and the risk of accidental injury at school among Korean adolescents.
METHODS
From the database of the Ninth Korea Youth Risk Behavior Web-based Survey (KYRBS), the researcher selected 63,307 adolescents who responded to a survey on sleep hours. We conducted logistic regression with sleep duration and fatigue after sleep as independent variables, the risk of injury at school as a dependent variable, and gender, grade, school type, economic status, parents' education level, number of participations in physical education, and current smoking and drinking as control variables.
RESULTS
Using 9 hours of sleep as the reference, the adjusted injury risk (odds ratio) was 1.74 for those sleeping less than 5 hours a day, 1.61 for 5 hours, 1.45 for 6 hours, 1.31 for 7 hours, 1.13 for 8 hours, and 1.40 for 10 hours or longer. The difference between each pair of groups was statistically significant. In this study, injury risk increased as sleep duration decreased and fatigue after sleep increased.
CONCLUSION
The findings suggest that a short nightly duration of sleep and fatigue after sleep can be considered potential risk factorsfor unintentional injuries at school among Korean adolescents.

PURPOSE
The aim of this study is to determine the impact of abuse experience and coping styles on suicidal ideation in Korean elders.
METHODS
Data were collected from the 2011 Korean National Survey on the Elderly. The participants were 10,537 elders aged 65 or older who had replied to the question on suicidal ideation. Socio-demographic and health related factors, abuse experience, and coping style were included as the influence factors of suicidal ideation. Data were analyzed using x2-test and logistic stepwise regression with the SPSS/WIN 21.0 program.
RESULTS
The prevalence of abuse experience among the elders was 12.7% and that of suicidal ideation was 11.2%. Elders with abuse experience (OR=2.79, 95% CI=2.39~3.26) were more likely to have suicidal ideation but those with an active coping style (OR=0.69, 95% CI=0.57~0.84) were less likely to have suicidal ideation even with other influence factors controlled.
CONCLUSION
Abuse experience of the elderly was a major risk factor that increased suicidal ideation, and active coping style was a protective factor that reduced suicidal ideation. These findings show that careful attention to elderly abuse and education for active coping style are necessary to reduce suicidal ideation among the elderly.

PURPOSE
The aim of this study is to clarify the level of health literacy, health risk perception and health behavior of Korean elders and to determine the impact of their health literacy and health risk perception on their health behavior.
METHODS
A descriptive correlation study was conducted with 188 elders aged 65 or older in senior welfare centers in Busan. Questionnaires were used to measure levels of health literacy, health risk perception, and health behavior. Descriptive statistics, t-test, ANOVA, Kruskal Wallis test, Pearson's correlation, and hierarchical regression were performed.
RESULTS
43.6% of the subjects had low levels of health literacy. There was a negative relation between health literacy and health risk perception, and between health risk perception and health behavior. There was a positive relation between health literacy and health behavior. Health concern, health literacy affected health behavior. Health literacy independently accounted for 24% of health behavior. Health risk perception didn't affect health behavior.
CONCLUSION
Many of the Korean elders had low levels of health literacy and health literacy was independently associated with health behavior. These findings show that interventions for improving health literacy are necessary to enhance health behavior of the elderly.

PURPOSE
The purpose of this study is to describe the prevalence of suicidal ideation and to investigate related factors contributing to suicidal ideation in elderly people with focus on comparison of gender differences.
METHODS
From the database of the Fourth Korean National Health and Nutrition Examination Survey (KNHANES-IV), the researchers selected 3,164 old adults aged over 65. The original study was a population-based epidemiological survey of health and nutrition in a stratified multistage clustered probability design. Data were analyzed by using chi2-test and t-test and multiple logistic regression with the SPSS/WIN18.0program.
RESULTS
Suicidal ideation in women (37.5%) occurred more frequently than men (17.5%). Depression, stress, restriction on activities and quality of life were significant variables of suicidal ideation in both the men and women aged 65 over. Low educational levels and economic states were significantly associated with suicidal ideation in men but not in women.
CONCLUSION
The development of a suicide prevention program for elders requires different approaches to the genders respectively. It is also recommended that programs be developed that can help control the variables identified in this study along with a follow-up study for verifying the model.

PURPOSE
The purpose of this study was to investigate factors associated with the intention to quit smoking in community-dwelling elderly smokers.
METHODS
From the database of the Fourth Korean National Health and Nutrition Examination Survey (KNHANES-IV), the researchers selected 448 elderly smokers over the age of 65. Data were analyzed with the chi2 test and multiple logistic regression using the SPSS/WIN 18.0 program.
RESULTS
Of the respondents, 45.5% intended to quit. The factors associated with the intention to quit were relative youth, having limit of activity day living and having past attempts to quit.
CONCLUSION
Population-based smoking cessation programs, especially those targeted at the elderly, should take these predictors into consideration in the design of interventions.

PURPOSE
The purposes of this study were to describe the prevalence of low weight and to investigate related factors contributing to low weight in community-dwelling older adults.
METHODS
From the database of the Fourth Korean National Health and Nutrition Examination Survey (KNHANES-IV), the researchers selected 2,964 older adults aged over 65. The original study was a population-based epidemiological survey of health and nutrition with a stratified multistage clustered probability design. Data were analyzed using chi2-test and multiple logistic regression with the SPSS/WIN 18.0 program.
RESULTS
The prevalence of low weight in older adults was 4.7%. The prevalence of low weight was different according to demographic characteristics. That is, the rate was higher in male elders, older ones, and rural residents. In the cases of current smoking, frequent alcohol consumption and no chronic disease, the prevalence of low weight was higher. Age, gender, area of residence, and current smoking were found to be contributing factors to low weight in community-dwelling older adults.
CONCLUSION
The result of this study provides important insights into possible reasons for low weight in community-dwelling older adults and suggests that low weight should be considered in promoting older adults' health.

PURPOSE
The purpose of this study was to analyze service occupation types and to develop training programs for the aged.
METHODS
This study used descriptive study design. The research process consisted of three stages: first, identified the demand of service manpower for the aged; second, investigated present jobs and education programs in Korea and Japan; and last, developed service jobs and education programs for the aged.
RESULTS
Potential users considered "health management" to be the most important area. They thought "providing job" as second most important. According to the result of analyzing Korean policies, there were 9 service occupations in 5 domains. So, we derived 10 occupations such as 'daily living manager', 'care manager', 'care worker', 'health manager', 'education specialist', 'leisure manager', 'good manager', 'housing manager', 'financial specialist' and 'retirement consultant' in 5 domains as healthcare, leisure, goods, housing and finance. Finally, we developed their tailored training programs.
CONCLUSION
According to this study, there should be various occupations qualified by the government, and training programs should be settled. And healthcare providers must included in developing standardized training programs.

PURPOSE
The aim of this study was to explore the health-related quality of life of caregivers for demented elders.
METHOD
The subjects of this study were 170 pairs of demented elders and their caregivers. Socio-demographic characteristics of the demented elders and the caregivers, Barthel index and SF-36 were used in this study. Data analysis procedure included t-test, ANOVA, Pearson correlation coefficient, and hierarchical multiple regression.
RESULTS
The score of overall health-related quality of life (HRQoL) was 288.62 in norm-based scoring. The major factors that affect HRQoL of the demented elders' caregivers were burden, the age of the caregiver, ADL and gender of the demented elder, and these factors explained 34.5% of HRQoL.
CONCLUSION
The factors significantly affecting the caregivers' HRQoL were burden, the age of the caregiver and ADL. The effective social support system should be considered in respective nursing interventions to decrease the level of burden and to increase HRQoL in demented elders' caregivers. Further studies and efforts will be needed to investigate preceding factors of burden and HRQoL.

PURPOSE
This study were to understand the caregiving situation of families with the demented elderly and to develop a self-help program for them.
METHODS
A self-help program was designed to increase caregivers' confidence and skills in problem solving. In addition, it was designed to provide information about the demented elderly. The self-help program of this study was composed of nine sessions.
RESULTS
Family caregivers of elderly patients with dementia obviously experience high level of stress. which often leads the caregivers to negative outcomes such as depression, interference with social activities and health difficulties. That is, caring is stressful for caregivers because the demands of the caregiving situation tend to overwhelm the caregivers' coping resources. Therefore, families with demented elderly persons need assistances in order to be relieved from their care burden.
CONCLUSION
The self-help program includes the understanding of dementia, the family caregiving experiences of the demented elderly persons, the time for self-care, the search for helping, the improving of interpersonal relations and communications.

PURPOSE
The purpose of this study is to grasp the welfare service for the demented people staying at home and the service that caregivers want to use. Therefore, we are going to develop a Korean senile dementia-care management model.
METHOD
It analyzed the data of 185 demented people and caregivers, who registered in 16 public dementia-care centers in B city since June 2002.
RESULTS
1) The types of services used by the aged people with dementia staying at home were, in the order of frequency of use, the day-care center (26.5%), and home-help service (21.6%). 2). The types of services according to the degree of dementia were as follows; mild cases: home-care service (5.4%), moderate cases: day-care service (40.0%) and severe cases: day-care service (26.0%). 3). The caregivers who want to use senile welfare institutions accounted for 23.3%, and the major reason they could not use the institutions was due to their economic situation. 4) The Korean senile dementia care management system must be executed, considering caregivers' economic state and severity of dementia. Since the system was actively operated, many small sized welfare service institutions showed development.
CONCLUSION
The welfare services appropriate to the severity of dementia should be provided. With the model developed in this study, the dementia management requires sufficient care and should be achieved to reduce the caregivers' burden.

PURPOSE
The purpose of this study was to identify the effects of the Sophrologic Prenatal Education Program.
METHOD
The subjects of this study were 16 antepartum women who had been registered at community health centers at S-gu in Pusan. Data were collected from women who visited for antenatal care during the period from the 7th of May to 28th of June in 2003. Participants were evaluated before and on completion of the 8 weeks' educational program using 3 instruments: Breast-feeding knowlege, Maternal-fetal attachment and Antenatal stress. The Wilcoxon Signed-Ranks Test was conducted for data analysis using SPSS/WIN 10.0.
RESULTS
Statistically significant difference was found between the pretest and posttest in the mean score of the Breast-feeding knowledge. The mean scores of the experiment group and the control group were 31.44+/-5.51 and 37.06+/-4.27 respectively (p=.001). No statistically significant difference was found between the pretest and posttest in the mean score of the Maternal-fetal attachment (p=.315, p=.578).
CONCLUSIONS
Community health care providers must consider the emotional needs of Korean people and make efforts to adapt the Sophrologie Prenatal Education Program for a larger number of antepartum women in community.
An Investigation on the Status of Care-giving for the Demented Elderly and Desire for In-house Care Services

PURPOSE
The purpose of this study is to examine the status of care-giving for the demented elderly and to find out the need of community in-house care services- day care services, short-term care services, home help services, and home care services.
METHOD
It analyzed the data of 186 old people having dementia, and caregivers. Data were collected for five days, in September 2002.
RESULTS
The caregivers were mostly women and the burden for the care giving was high (87.5%). They used community care services, that is, day care services (26.5%), home care services (21.6%), home help services (10.8%), short-term care services (6.2%). Caregivers' age and education level were significant factors in the demand for day care services. Caregivers' education level was a statistically significant factor in the demand for short-term care services and home help services. Caregivers' age and education level were significant factors in the demand for home care services.
CONCLUSIONS
It is necessary to expand the financial aid for the active implementation of daytime protection for dementia-patients under medical treatment at home and to promote patients' recovery. It is necessary to enhance home help services and home care services, and to establish many day care centers and short term care centers. Through this, it will prevent caregivers from becoming burnt out due to the burden of care giving.
A Comparative Study on the Self-Efficacy and Health Promoting Behavior between Obese and Normal Weight Middle School Students

PURPOSE
This study was aimed to identify the difference of the self-efficacy and the health promoting behaviors between obese and normal-weight middle school students.
METHOD
Data were collected from a convenient sample of 360 middle school students (180 normal-weight students and 180 obese students) in Pusan from November 22nd to December 20th, 2000, based on self-report questionnaires. The following instruments were used in the study after being reconstructed by the researcher with the consultation of experts and the literature review: General Self Efficacy Scale(Sherer et al., 1982), and Health Promoting Lifestyle Profile (Walker, Sechrist & Pender, 1987). The data were analyzed with descriptive statistics, chi-square test, and t-test and Pearson's correlation coefficient.
RESULTS
1. There were significant differences in father's obesity (p=.014), mother's obesity (p=.007), sibling's obesity (p=.001), and school record (p=.001) between obese and normal-weight students. 2. There were statistically significant differences in the score of self-efficacy between the obese and normal-weight students (p=.000). 3. There were statistically significant differences in health promoting behavior (p=.000) and in six sub-items: personal relationship (p=.000), exercise (p=.001), responsibility for health (p=.003), stress management (p=.015), self-actualization (p=.043) between the obese and normal weight students. 4. There were very statistically significant negative correlations between obesity and the self-efficacy (r=-.19, p<.001), health promoting behavior (r=-.18, p<.001), specifically personal relationship(r=-.20, p<.001), stress management (r=-.12, p<.05), responsibility for health (r=-.15, p<.01), and self-actualization (r=-.14, p<.01).
CONCLUSION
According to the above results it is concluded that self-efficacy and health promoting behavior influence the obesity of middle school students. Therefore I recommend adding programs that can lead to improve self-efficacy and health promoting behavior to the obesity prevention programs for middle school students.

PURPOSE
This study was aimed at investigating the recognition and attitude of inpatients for the purpose of facilitating the implementation of the home health care program in relation to the opening of the Home Care Department in P University Hospital in Pusan.
METHOD
Data were collected from the questionnaires returned by 293 patients, who were admitted at P University Hospital, from May 1 to May 15, 2001 and analyzed using descriptive statistics and the Fisher exact test.
RESULTS
In regard to the previous information about the home health care program, 55.6% of the inpatients heard about the name through the advertisement in the hospital, the mass communication, and from acquaintances. The percentage of right answers regarding the knowledge of home health care program was 43.9%. In regard to the acceptance of the home health care program, 97.1% of patients agreed with the implementation of the home care program. The reasons for acceptance were: the maintenance of the continuity of care, the alleviation of the family burden of time.
CONCLUSION
For the stabilization and successful implementation of a home health care system, it should be accompanied not only by advertisement but also education for patients about the home health care program, and a post-evaluative study after the implementation.
